Notarization Requirements
1. The Signer must appear in front of me for the Notarization with proper ID (see below)
2. If the document is a Sworn Document, it must be signed in front of me, if unsure, dont sign the document until I get to the appointment.
3. All Signers must show acceptable identification, ID must contain a picture, a signature, and must be current.  In some cases, 2 ID's may be required.
Acceptable ID's Are:
*State Issued Drivers License
*State Issued Identification Card
*Military Identification Card
*Passport
4. Blanks within the document should be completed before I arrive to the appointment.
5. The proper Notarial verbiage should be present on the document (see here for sample statements). If not, I can provide the proper certificate by your direction.
6. If Witness's are required for your document, please make arrangements to have them present at the appointment.  They may also need to provide proper identification.
